ALL patients must supply our office with the necessary insurance information prior to their 1st appointment which consist of the name of the insurance company, the employer, the employee, the employee's ID number, and the employee's date of birth. If a secondary insurance is available that information will also be needed to properly bill the insurance company for you. If a current copy of the insurance card is available we will make a copy of it for your records.  We will fill out your insurance forms and mail to your insurance company for you. Any deductibles and/or co-payments are due at the time of treatment. A 5% professional courtesy is available to those patients who do not have dental insurance and pay for treatment in-full AT THAT TIME with a check or cash. We also accept MC/VISA and DISCOVER but a  discount is not available when using a credit card.
